This Privacy Notice explains what type of personal information we may collect from you and how we use it when you enrol in a Takeda Access, Use or Support Program (the “Program”).

The Program is developed by Takeda Pharmaceuticals Australia Pty Ltd (“Takeda”) and may be managed by our providers (“Provider(s)” or “we”). The Program aims to provide support and/or access for Australian based patients regarding their condition and/or treatment.

How your personal information is collected and used

We collect and use personal information about you as may be required for the purposes of the Program, and to respond to or fulfil your requests. The personal information we may collect includes, but is not limited to, your contact information, delivery address, and sensitive information such as your (or your patient’s) health information, including treatment, and treatment schedule, and any other information relevant to participation in the Program. As applicable, the personal information may be provided by you (or your parent or guardian), or collected from healthcare professionals involved in your treatment, including your treating doctor, nurses and/or pharmacists. We may not be able to provide you services under the Program if your personal information is not collected.

Contacting you with Program information

As part of agreeing to participate in the Program, and to this Privacy Notice, you consent to receiving proactive information about the Program and related material (which may include information on Takeda products where relevant to your participation in, or engagement with, the Program).

Sharing your personal information

Information about your participation in the Program may be shared with others, such as any healthcare professionals involved in treatment, or the partners we work with to provide this Program and the services under it. We may also use or disclose your personal information as needed to comply with regulatory requirements. This includes reporting of safety information to relevant regulatory authorities and/or Takeda.

Information about you will generally only be shared with third parties in de-identified, aggregated reports for the purpose of Program administration and review, monitoring, health technology assessments and research. 

Storing your personal information

We store your personal information in Australia. The sharing of personal information as described above may involve transfers to other countries. The data protection and privacy laws in such countries may provide a different level of protection. We will put in place appropriate safeguards to protect personal information in connection with such transfers.
